1. Look at our Qualifications page, located under the Apply tab. Decide whether or not you qualify.
2. Fill out an application.
3. Upon filling out the application, you will need to provide all of the following documents by emailing them to :
Proof of vehicle registration (name on registration must match either Applicant's name OR name listed as vehicle owner on the application)
An estimate of the cost of modifications from two certified mechanic shops near you
Copy of Applicant's valid driver's license, front and back
Doctor's Statement including: brief history of your injury, including when, how, and what level of injury was sustained; current medications/treatment program; doctor's medical office contact information
Letter of Necessity explaining why you need modifications
Proof of Income (SSDI annual report, W2, or first two pages of most recent tax return)
